Instructions for uploading your images:
Format all images in the following way:
1. Save your images as JPEGS.
2. File size should be less than 1MB, no larger
than 2000 pixels on any side.
3. Before you upload your images, save each
individual image as:
FirstLastName1.jpg (Number corresponds to each
image 1-5.)
After
successfully uploading each image you will see the upload page.

After you have submitted your information and images, you can pay online using a credit card:
